Dementia Clinical Pathway Group (CPG) Metrics: West Midlands, Clinical Measures

Summary

Dr Karim Saad, the West Midlands’ Dementia CPG Clinical Lead explains the tasks and challenges involved in measuring progress in the implementation of the regional dementia pathway.

Innovative new metrics are presented. The priority has been to select metrics which score highly on validity and responsiveness, while attempting to avoid perverse incentives and reduce the cost of data collection. The measures used so far include:

  1. Dementia Clinical Pathway Group (CPG) Metric 1: Early Diagnosis.
    1. Ratio of GP recorded prevalence to estimated population prevalence, 2007/08 (PCT).
    2. Ratio of GP recorded prevalence to estimated population prevalence, 2007/08 (SHA).
    3. Ratio of GP recorded prevalence to estimated population prevalence (West Midlands Average).
  2. Dementia Clinical Pathway Group (CPG) Metric 2: Crisis Prevention.
    1. Emergency admissions for dementia per 1,000 persons with dementia, Apr-Jan 2008/09 (PCT).
    2. Emergency admissions for dementia per 1,000 persons with dementia (West Midlands Average).
  3. Dementia Clinical Pathway Group (CPG) Metric 3: Patient Experience in Secondary Care.
    1. Excess bed days for patients with dementia per 1,000 persons with dementia, Apr-Jan 2008/09 (PCT).
    2. Excess bed days for patients with dementia per 1,000 persons with dementia (West Midlands Average).
  4. Dementia Clinical Pathway Group (CPG) Metric 4: Coordinated Care.
    1. Delayed transfers of care per 10,000 population 65+, Apr-Feb 2008/09 (Local Authority).
    2. Delayed transfers of care per 10,000 population 65+ (West Midlands Average).
  5. Dementia Clinical Pathway Group (CPG) Metric 5: Access to Treatment.
    1. Access to treatment.
  6. Dementia Clinical Pathway Group (CPG) Metric 6: Support for Carers.
    1. % of all clients receiving community-based services, 2007/08 (Local Authority).
    2. % of all clients receiving community-based services, 2007/08 (Region).
    3. % of all clients receiving community-based services, 2007/08 ( West Midlands Average).
  7. Dementia Clinical Pathway Group (CPG) Metric 7: Support for Carers.
    1. Support for carers.
  8. Dementia Clinical Pathway Group (CPG) Metric 8: Investment.
    1. Net expenditure per 100,000 population for 5b organic mental health disorders, 2006/07 (PCT).
    2. Net expenditure per 100,000 population for 5b organic mental health disorders, 2006/07 (SHA).
    3. Net expenditure per 100,000 population for 5b organic mental health disorders, 2006/07 (West Midlands Average).
  9. Dementia Clinical Pathway Group (CPG) Metric 9: End of Life Care.
    1. Proportion of all deaths from dementia occurring in an acute setting, 2006/07 (PCT).
    2. Proportion of all deaths from dementia occurring in an acute setting, 2006/07 (West Midlands Average).
