Dementia Services Guide (Healthcare for London)

Summary

The Healthcare for London Dementia Services Guide was produced in 2009 to provide advice to London commissioners, and to clinicians working with local authority partners, about how to follow the integrated care pathway for dementia (developed by Healthcare for London).

The aims of this document are to provide:

  1. Assistance to London commissioners when planning services in partnership with local authority colleagues in social care.
  2. A quality check against which services can be benchmarked.
  3. Performance outcomes for use in reviewing services.

This guide includes sections on (a) the integrated care pathway, (b) the general hospital care pathway and (c) memory services.
