Many people are looking for the holy grail of a complete list of Clinical Commissioning Groups (CCGs). CCG configuration is still in progress and there is currently no official source for mapping practices to CCGs.
The Association of Public Health Observatories (APHO) has been compiling their attempt at a comprehensive list. This list is a work in progress.
This list maps Clinical Commissioning Groups in the National General Practice Profiles. The list (actually an Excel workbook comprising several worksheets) is claimed to be as up-to-date as possible, but APHO are aware that it is neither perfect nor final. It is updated regularly.

Note: The NHS Commissioning Board Authority (now NHS England) has more recently published their own list covering the proposed configuration and member practices for 212 proposed CCGs.
CCG Funding Allocation Growth Projections 2016/17 to 2018/19 (NHS England)
Commissioners have been supplied with funding allocation estimates to help them develop their five years strategic plans. CCGs can project growth in their funding allocations.
