Narrative on Person-Centred Coordinated (“Integrated”) Care (NHS CB / National Voices)

Summary

The NHS Commissioning Board (NHSCB) commissioned National Voices to develop a narrative for integrated care from the viewpoints of patients and service users. This was done in partnership with the Local Government Association with support from the Department of Health and Monitor

Note: Feedback and comments on person-centred and coordinated (‘integrated’) models of care are welcome until February 28th 2013.
