“This is Me” Tool (Alzheimer’s Society)


The “This is Me” information tool supplies staff with summary information about the person with dementia as an individual, with unique needs, preferences, likes, dislikes and interests.

The “This is Me” leaflet has been updated to give it wider relevance for people with dementia receiving professional care in any setting; whether at home, in hospital, in respite care or in care homes. It was originally developed for people with dementia admitted into hospital.
