SBAR Tool: Situation – Background – Assessment – Recommendation (NHS Institute for Innovation and Improvement)


The SBAR tool consists of standardised prompt questions to frame communication, in four sections. The steps are:

S: Situation

B: Background

A: Assessment

R: Recommendation
