NHS England has created a set of tools to enable GPs, practices and CCGs to monitor and improve dementia diagnosis rates and the follow-up care and support for people with dementia. Effective use of these tools requires some explanation. The necessary documentation is available via the Dementia Partnerships Knowledge Portal.
“The Dementia Prevalence Calculator enables General Practices and Commissioners to plan and take action to improve local diagnosis rates using a range of mechanisms, including improving coding, case finding, targeted screening, and education to improve recognition, assessment, and diagnosis”.
