Transitions Between Health and Social Care (NICE)

Summary

The National Institute for Health and Care Excellence (NICE) is developing a social care guideline on coordinated transitions between health and social care, for use in England. The draft scope of this guideline on integrated care defines the aspects of care to be covered and the people / organisations involved. The deadline for feedback to this scope consultation is February 11th 2014.
