YHPHO’s Better Care Fund: Related Metrics Forecasting Tool

YHPHO is now part of Public Health England, and has released an Excel-based forecasting tool. This tool forecasts BCF-related metrics for Health and Wellbeing Boards involved in the BCF planning process. The aim is to help health and local authority commissioners to access data by area, to improve patient care pathways and the demand for non-elective hospital admissions.

“The tool has forecasts for total non-elective admissions by Health and Wellbeing Board and for avoidable emergency admissions and associated metrics. Delayed transfers of care are also included. The tool assists those working with Health and Wellbeing Boards on the Better Care Fund BCF plans. The tool helps to inform plans by presenting specific trajectories associated with avoidable emergency admissions and set the level of reduction in the number of non-elective admissions”.
