The NHS in Staffordshire has received £300m to implement these changes.
Stafford Hospital will be renamed as the County Hospital. The City General will be re-named as the Royal Stoke University Hospital.
The Mid Staffordshire NHS Foundation Trust is to be dissolved next month, after having been in administration since April 2013. Some services will move from Stafford, under control of the Royal Wolverhampton NHS Trust.
