End of Life Care for People Living With Dementia in Walsall Care Homes (Housing LIN)

Summary

The following Housing LIN case study covers end of life care services in Walsall, provided by a collaboration between Pathways 4 Life (itself a partnership between the Accord Group and Age UK Walsall) and St Giles Walsall Hospice. The creation of safe and relaxing spaces, and provision of meaningful person-centred activities, are key features which enhance quality of life.

Two community-based Dementia Support Workers (DSWs) work in care homes across Walsall and provide the expertise for improving dementia and end of life care. These DSWs assist and empower care home staff, people with dementia and their families, using the Namaste Care approach. Adoption of a holistic approach to dementia care is encouraged.
